Experience with primary liver transplantation across ABO blood groups.

چکیده
THE LIVER has long been regarded as a privileged organ which can be transplanted across incompatible ABO blood groups with little risk of hyperacute rejection. l However, in a recent review of 671 first, second, and third liver transplants we found a significant advantage for ABO donor-recipient identity for primary liver transplants.2 Although a large number of ABO mismatched grafts were successful, graft survival for primary liver grafts between ABO identical donor-recipient pairs was significantly better than grafts between ABO compatible but nonidentical or ABO incompatible donorrecipient pairs. The extent to which urgency of transplantation influenced these results was not fully assessed. An urgent factor existed in many of the transplants between ABO mismatched donor-recipient pairs. We here report a review of 745 primary liver transplants performed between Jan I, 1981 and Dec 31, 1986 at the University Health Center of Pittsburgh in which the early outcome of primary liver transplantation across ABO blood groups was assessed in relationship to the urgency of transplantation.

Antibody-mediated rejection of human orthotopic liver allografts. A study of liver transplantation across ABO blood group barriers.
A clinicopathologic analysis of liver transplantation across major ABO blood group barriers was carried out 1) to determine if antibody-mediated (humoral) rejection was a cause of graft failure and if humoral rejection can be identified, 2) to propose criteria for establishing the diagnosis, and 3) to describe the clinical and pathological features of humoral rejection. A total of 51 (24 primar...

Six hundred seventy-one first, second, and third orthotopic liver allografts in 520 patients were reviewed to determine the effect of donor-recipient mismatches or incompatibilities for the ABO blood groups on graft survival. A significant advantage for ABO donor-recipient identity was found, especially in adults and for first grafts. However, a surprisingly large number of ABO incompatible gra...

BACKGROUND & AIMS Living-donor liver transplantation (LDLT) can simultaneously cure hepatocellular carcinoma (HCC) and underlying liver cirrhosis, improving long-term results in patients with HCC. ABO-incompatible LDLT could expand the living-donor pool, reduce waiting times for deceased-donor liver transplantation, and improve long-term survival for some patients with HCC. METHODS We retrosp...

Blood-group type matching has been well known as the principal rule in organ transplantation. However, as the possibility of obtaining grafts from living donors is very low, adjustment of blood type is sometimes very difficult. On the other hand, and particularly in Far East countries, it is even harder to obtain grafts from deceased donors; thus, living donors must be used for the rescue of pa...

L IVER ALLOGRAfTS are resistant to hyperacute or antibody mediated rejection from preformed antibodies. No consistent syndrome develops following transplantation when a recipient harbors preformed Iymphocytotoxic antibodies.1.2 The only apparent deleterious effect of preformed Iymphocytotoxic antibodies is a higher incidence of the "vanishing hile duct syndrome.") Despite these clinical observa...
